Why is the top of my foot swollen with no injury?

I woke up with my foot puffy and swollen on top, but I haven't twisted or injured it at all. It gets slightly better when I elevate it, but the swelling always comes back. Why is this happening, especially since I'm not overweight, and is it a sign of a serious medical problem or just bad circulation?
